These forms must be filled out, signed, and mailed or delivered to the Child's Day office at least one week before the first day of attendance:

   Child Release and Emergency Contacts List
    Health Record
      Care Instructions & Background Information
      Authorization to Provide Medical Report
    Physician's Medical Report  (can follow within 2 weeks)
